In the past I entered the CDG number and then just selected the "next" book ID number available but it won't move from the number "1". It also will not let me change the "1" to any other number. ??? When I upgraded recently, I had not upgraded in about 5 years so maybe that's why all the crap is happening.

Did I understand right that I need to downgrade and then rebuild? I am running XP.

Since you have bought the upgrade, you can downgrade to version 4.32 which you can download from the Support section of the website. Again be sure to delete your Songs.mdb file and remove 5.04 or 5.05 before you install 4.32.
The only reason the BookID should be stuck at that number should be that the particular CDG DiskID you entered to import was already in the database.
