Wes, if you are using ZIP files or KMA HDZIP type files, then there was a bug in 4.003 that duplicated the last ZIP song for all other ZIP songs in that Write Disc List.
It is documented here.
The other friends answering here are also right; you need to update your Burner drivers and all Windows CUSTOM Updates.
