I don't think that was quite what Jim meant by full screen. I don't have my computer with the Nvidia, so I can't look at the settings for sure. But I do know he wasn't referring to running the programs at full screen.
You stated you had the latest drivers install so that is why I mentioned it. Even the original drivers may cause problems. You may have to try several to find the one that works right.
You could try changing the DirectX acceleration and see if that will solve the initial sync problem. To do this go to Start/Run and type in DXDIAG. This will open a utilty where you can lower the amount of acceleration.
I don't know why keying up or down would be any different from normal play. All that does is shift the tone up or down and does have anything with the speed (tempo?) or sync.
