Since you haven't checked the Custom Updates since 4.06 I would suggest you do look at it and add any new ones. The one Sam was referring to that fixed his may help yours.
I do have dual cards and will check to see if I can duplicate this. I am not sure I would call this a bug, but I guess you can get in a hurry and cause it to happen if you don't stop the preview before closing.
